SHIPPING
The dental office may use any method they choose to ship cases to our laboratory.

Shipping charges to send cases to the laboratory are the responsibility of the dental office.
Return shipping to the dental office is provided at no charge by Malcamp Dental Ceramics.
SERVICE SCHEDULE
Completed restorations will be returned to your office approximately 15 working days (3 weeks) from the date the case arrives in our laboratory. Make the appropriate adjustments for any holidays or vacations. Large or complex cases and implant cases will require additional time. Please call in advance to preschedule these cases.
Do not count pick-up day, days in-transit or the delivery day as working days when scheduling your cases.
Avoid scheduling patients on the same day the case is due back in your office.
Call us in advance to schedule a rush case. Additional service fees and/or shipping charges may apply to rush cases.
Anterior multi-unit cosmetic cases typically require additional time in the laboratory. Advanced scheduling of these cases can provide a definitive return date, generally about 4 weeks from the day all items necessary for the completion of the case are received in the laboratory. Otherwise the case will be entered into the work schedule according to the earliest time slot available that allows for the proper length of time necessary for completion of the case. All necessary components must be delivered to the laboratory before we can begin the case or set a confirmed delivery date.
PAYMENT TERMS
An itemized invoice will accompany each case. Statements are prepared and mailed out at the end of each month. Payment terms are net 15 days from the statement date. We appreciate full payment at this time. A 2% service charge will be added to unpaid balances if a payment is not received by the 25th of the month.
Accounts that are past due by the end of the next billing cycle will be converted to a C.O.D. basis.
REMAKE POLICY
Occasionally a case requires adjustments or a remake is needed. Remake orders must include all the original models and the crown, plus a new impression. The remake case is closely evaluated to determine the cause of the remake. Additional charges may be incurred to complete the remake.
Credit for alloy used to make the new crown is given only when the original crown is returned with the case.
When porcelain needs to be added to close a proximal contact, please return all models with the crown. A new impression is helpful in determining where and how much porcelain to add to close a contact. If a new impression is not taken, the solid model should be adjusted to reflect the gap that needs to be filled to close the contact.
Prior to cementation, if a remake is necessary due to a change in the original shade ordered, there will be a charge for stripping off porcelain and rebuilding the crown.

- Selected shade was changed after the restoration is cemented in the mouth.
- Open margin where the dentist has approved impression or trimmed margins on die.
- Restoration remake due to distorted impression, inaccurate bite records, or fractures due to debonding.
- We requested a try-in and you asked us to proceed without one.
- We received no study model for an anterior case, and/or no specific direction was given, and/or no photos were sent.
